It takes many different medical disciplines to work with a patient. Everyone has different areas of expertise. All of these disciplines combine to give patients the best possible care with the outcomes that best work for the patient. When the care plan team (also called the interdisciplinary team) they collaborate, with the patient and family, to ensure this goal will be achieved.
